Short Questionnaire on Stroke as it Pertains to Women

Dear fellow nurses,
I am a nurse practitioner student at Millersville University working on my scholarly project. Recently the American Heart Association/American Stroke Association released guidelines for stroke prevention in women. This is the first gender specific stroke guidelines to date. Based on these guidelines my project involves a needs assessment to determine the educational deficits and needs of healthcare providers in the prevention of stroke risks in women.
I have prepared a short questionnaire that asks your opinion on stroke as it pertains to women. I am requesting for all questionnaires to be completed by April 7, 2014, by midnight. Please utilize the provided link to access the questionnaire. https://www.surveymonkey.com/s/S58KCPT
