Family Nurse Practitioner in Need of Primary Care Rotation this Fall

Hello!

My name is Toni, I am a Family Nurse Practitioner student at Samford University, and am in need of a primary care rotation with an FNP this Fall. I would love to lend a hand at your practice, and am willing to help in any way I can to make your workday go smoothly. I live in Philadelphia, but am absolutely willing to travel.

This will be my fourth clinical rotation, so I have plenty of experience establishing a clinical site and preceptorship. Completing the paperwork is my responsibility, I would just need your signature on the Preceptorship Planning Form and a Clinical Affiliation Agreement. The malpractice offered through my school is detailed on page 26 of the Clinical Affiliation Agreement. Furthermore, the contract can be broken at any time if you decide having a student isn't working out for you.

Attached is my resume, the mission statement and history of Samford University, the course objectives for this clinical rotation, as well as a blank copy of the Preceptorship Planning Form and Clinical Affiliation Agreement.

If you have any questions, please don't hesitate to ask! If you are unable to precept, but have another FNP in mind, that would be wonderful, too. Any help with this would be greatly appreciated, thank you for your time!
