Position Description

An innovative kidney care program is seeking experienced Nurse Practitioners (AGNP, AGACNP, FNP) to deliver primary care for patients with Chronic Kidney Disease (CKD) or End-Stage Kidney Disease (ESKD) across Baltimore and Towson, Maryland. This temp-to-perm opportunity allows providers to help build and scale a new, value-based model of integrated nephrology care.

NPs will collaborate closely with nephrologists and dialysis teams, managing complex chronic conditions while working to reduce hospitalizations and improve long-term outcomes. This is a weekday-only role with leadership growth potential.

Key Responsibilities

• Provide primary care within dialysis units or nephrology clinics

• Manage CKD/ESKD patients with complex comorbidities

• Coordinate care with nephrology and interdisciplinary teams

• Reduce avoidable hospitalizations through proactive management

• Support development of new outpatient CKD clinics

• Participate in value-based, population health initiatives

Qualifications

• Active NP license in Maryland (DE & PA a plus)

• 4+ years NP experience (nephrology preferred)

• Experience managing HF, HTN, DM, and complex chronic illness

• DEA & BLS required

• Independent, growth-oriented mindset

Schedule & Compensation

$90–$95 per hour

• Monday–Friday | Five 8-hour shifts

• No nights, weekends, or call

• Temp-to-perm opportunity
