Job Title: Advanced Practice Clinician - Nurse Practitioner/Physician Assistant

Program: ISNP

Hours: 40 per week

Duration: Approximately 7-8 months

Job Description

The Senior Community Care (SCC) program is a longitudinal, integrated care delivery initiative that coordinates clinical care for members in a facility setting. The Advanced Practice Clinician (APC) within SCC provides care to our highest-risk health plan members as part of an interdisciplinary team.

Primary Responsibilities

• Conduct comprehensive assessments
• Manage acute and chronic medical and behavioral conditions
• Ensure accurate and complete documentation, including ICD-10 coding
• Communicate and collaborate with the interdisciplinary care team
• Lead advanced illness and advanced care planning discussions
• Provide patient and caregiver counseling and education
  
  

Required Qualifications:

• Licensure: Active and unrestricted license in residing state (or ability to obtain by start date)
• Education:
• Nurse Practitioner (NP): Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Physician Assistant (PA): Graduate of an accredited Physician Assistant degree program
• Certifications:
• NP: Board-certified by AANP or ANCC in Family, Adult, Gerontology, or Adult-Gerontology Acute Care
• PA: Board-certified by the National Commission on Certification of Physician Assistants
• DEA Licensure: Active prescriptive authority
• Physical Requirements: Ability to lift 30-pound bag, navigate stairs, and perform physical assessments
• Transportation: Reliable access and ability to travel between facilities
• Collaborative Practice Agreement: Ability to obtain, if applicable
• COVID-19 Vaccination: Mandatory per regulations
• Drug Screening: Required before employment

Preferred Qualifications:

• Experience:
• 1+ year of clinical practice (long-term care setting preferred)
• Managing complex behavioral, social, and functional patient needs
• Understanding:
• Geriatrics, Chronic Illness, Advanced Illness, and End-of-Life Care discussions
• Technical Skills:
• Proficiency in electronic medical records documentation

Schedule & Work Environment:

• Shift: Monday-Friday, 8 AM - 5 PM
• Dress Code: Medical attire (Scrub color: N/A)
