The ideal candidate will work Monday through Friday, 8 AM to 5 PM. This role involves providing care in a long-term nursing home facility, managing acute and chronic conditions, developing treatment plans, and monitoring for early changes in patients' conditions. Responsibilities include conducting scheduled follow-up visits, monthly checkups, acute care, admissions, and discharges for a diverse patient population, including some psychiatric and substance abuse patients.
