Nurse Practitioner:

Setting: Primary Care (Medicare Advantage Value Based Model)

Shift: Monday - Friday 8am - 5pm

Location: Hiring for multiple locations in the Cleveland Ohio area

Pay: $125,000 - $160,000+/annually, depending on experience

Population: Senior/Geriatrics

Start: June/July, direct hire position

Qualifications:

• BLS or ACLS Certification required
• NP License in state or on way to getting it
• FAMILY NP (not hospital NP)

Responsibilities:

Value Based Care Model (allotting 45 min with each patient!), Serving the senior population. Goal is for providers to build relationships with patients. Work alongside a doctor.

• Prescribe Medicine and work through non-complex cases
• Full schedule:
• 8 am team huddle
• 8:30 first patient
• 12-1 lunch break
• 3:30 last patient
• end of day admin
